Sheena Stelmack will be with us every Monday 9-5pm starting October 25th, 2021.

Dr. Hardik Yadav is with us every other Friday for appointments. However, he will be traveling for the entire month of December.

Please note that most appointments are scheduled around 6 weeks out.

The Five Rs are: right drug, right route, right time, right dose and right patient.

How We'll Help

Follow Ups: appointments scheduled out to continually assess how the medication is working for the client.

Unique Approach: no one patient is the same. We strive to look at our patients as unique individuals. A diagnosis of depression can affect and impact people very differently. No medication or solution is one-size-fits-all.

Problem Solving: Discuss and implement ways to solve the family’s most pressing problems through activities and insight.

Coordination of Care: between PCP and therapists (at our practice or otherwise!)

REMINDER ABOUT MEDICATION MANGEMENT POLICES.

 

Sheena Stelmack PMHNP only works Monday (In person/Telehealth) Tuesday (Telehealth only)

 

Prescription refill requests require 7 days notice.

 

Client’s on controlled substances (adderall, vyvanse, clonazepam ETC) are required to be seen every 30 days

 

Client’s on other medications need to be seen every 90 days unless otherwise discussed with the provider.

 

Requests for letters, forms etc require 14 days notice and there is a charge of $25-$50 per letter/form.

 

DISABILITY, ACCOMMODATION FORMS ETC WILL ONLY BE COMPLETED FOR CLIENTS SEEN FOR 6 MONTHS OR MORE ON A CONSISTENT BASIS.

 

LETTERS WILL NOT BE WRITTEN FOR EMOTIONAL SUPPORT ANIMALS.

 

IT IS AT THE DISCRETION OF THE PROVIDER AND PRACTICE OWNER, AND UNDER THE GUIDANCE OF OUR MALPRACTICE INSURANCE IF THEY CAN COMPLETE A FORM OR NOT.

EMAILS ARE NOT MONITORED 24/7 so please do not email urgent requests or detailed medical issues.

 

WE DO NOT HAVE 24/7 CRISIS CARE so clients need to go to their nearest emergency room or call 911.
